I, the undersigned volunteer, acknowledge that I have been given the opportunity to participate in a volunteer activity organized by Amigos Together For Kids, Inc., doing business as Amigos For Kids (“Amigos For Kids”).

In consideration of the benefits and opportunities provided through my participation, I voluntarily agree to the following terms:

Assumption of Risk and Release of Liability
I understand and voluntarily assume all risks associated with my participation in this program. This includes travel to and from program sites. I release, discharge, and hold harmless Amigos For Kids, its officers, employees, board members, agents, and sponsors from any and all claims, demands, causes of action, or liabilities of any kind. This includes damages, injuries, attorney’s fees, or other costs that may arise from or relate to my participation in this volunteer activity.

Indemnification
I agree to indemnify and hold harmless Amigos For Kids, including its board, staff, and sponsors, from any injury, damage, or loss I may incur during or after the event. This includes any injury, damage, or loss caused in whole or in part by any guest or participant.

Medical Authorization
I authorize Amigos For Kids, its employees, agents, or designated chaperones to obtain emergency medical treatment on my behalf if needed during my participation in the program. I agree to be financially responsible for any medical costs incurred.

Media Release
I grant permission for Amigos For Kids to photograph, film, or record me during volunteer activities. I understand that these materials may be used for promotional and publicity purposes such as newspapers, television, social media, and organizational publications. I acknowledge that I am not entitled to compensation for the use of these materials.

By signing below, I confirm that I have read and understood this waiver. I agree to its terms voluntarily and without reservation.
